Applications can be completed from any computer. If you are having problems with the online application process, please call the WHA office at 262-542-2262 between the hours of 8:00 am and 4:00 pm. A staff member will return your call as soon as someone is available to assist you.

TO COMPLETE AN APPLICATION YOU WILL NEED:

• Social Security numbers and dates of birth for all on your application
• Legal address of current residence
• Income information, all sources (i.e. wages, child support, social security benefits, etc.)
• Banking information and investment information
  
• Criminal history information
  
• A valid email address
